CLINICAL SUMMARY

Zopiclone (brand name Zopisign®, Imovane®) and Eszopiclone (brand name Lunesta®) belong to the cyclopyrrolone family of non-benzodiazepine GABA-A receptor modulators (“Z-drugs”). Zopiclone is a 50:50 racemic mixture of two stereoisomers ((S)- and (R)-enantiomers), while eszopiclone consists exclusively of the pharmacologically active (S)-enantiomer. Both agents significantly reduce sleep latency and nocturnal awakenings without substantially altering slow-wave sleep architecture. However, both carry a strict FDA Boxed Warning for Complex Sleep Behaviors (sleepwalking, sleep-driving, sleep-eating), must be avoided with alcohol or CNS depressants, and require conservative dosing to avoid next-morning psychomotor impairment and bitter metallic dysgeusia.

02. What are zopiclone and eszopiclone?

Zopiclone and eszopiclone represent the third major category of non-benzodiazepine sedative-hypnotics, chemically distinct from the imidazopyridines (such as zolpidem / Ambien®) and the pyrazolopyrimidines (zaleplon / Sonata®). They belong to the cyclopyrrolone chemical family.

When original zopiclone was synthesized, it contained two mirror-image molecules in equal parts: the (S)-isomer and the (R)-isomer. Receptor binding assays demonstrated that virtually all clinical sedative and hypnotic efficacy resides entirely in the (S)-enantiomer; the (R)-enantiomer is inactive at GABA-A receptors. In 2004, the isolated (S)-isomer was brought to the U.S. market under the generic name eszopiclone (Lunesta®), allowing targeted therapeutic sleep induction at approximately half the milligram dose of racemic zopiclone.

Global Regulatory Insight: Racemic zopiclone (Zopisign®, Imovane®) is one of the most widely prescribed hypnotics worldwide across the United Kingdom, European Union, and Canada. In the United States, eszopiclone is the approved version. Both agents share identical receptor binding pharmacology, efficacy profiles, and safety requirements.

03. Uses and approved indications

Both cyclopyrrolone hypnotics are prescribed for transient and chronic sleep disorders:

INDICATION CATEGORYCLINICAL OBJECTIVE & EFFICACY PROFILE
Sleep-Onset InsomniaDramatically shortens sleep latency (time required to fall asleep), typically facilitating sleep within 15 to 30 minutes of administration.
Sleep-Maintenance InsomniaBecause of their intermediate 5- to 6-hour half-life, both agents significantly reduce nocturnal awakenings and increase total sleep duration compared to ultrashort-acting agents.
Transient Insomnia (Shift Work / Travel)Short-term management of situational acute insomnia triggered by schedule disruptions or situational distress.

04. How do cyclopyrrolones work?

Patient-Centered Biological Explanation

When you prepare for sleep, specialized sleep centers in your hypothalamus release GABA, the brain’s master calming messenger. GABA attaches to inhibitory receptors on wake-promoting nerve cells, quieting brain chatter and allowing you to drift into sleep. When insomnia disrupts this natural process, wakefulness centers stay hyperactive. Zopiclone and eszopiclone bind with precision to the GABA receptor complex, holding the gate open to let negative chloride ions flood the nerve cells. This selectively shuts down the brain’s internal wake engine, triggering natural sleepiness without disrupting deep restorative sleep cycles.

Advanced Neuropharmacology & Subunit Binding

At the molecular level, cyclopyrrolones operate as positive allosteric modulators (PAMs) at pentameric GABA-A receptor complexes:

  • Binding Pocket: They bind to a specialized pocket distinct from classical benzodiazepines, located at the α1, α2, and α3 subunit interfaces with the γ2 subunit.
  • Sedative vs. Anxiolytic Profiles: Modulation of α1-containing GABA-A receptors mediates rapid sleep induction, while interaction at α2 and α3 subunits contributes to sleep maintenance and mild muscle relaxation.
  • Hepatic Metabolism via CYP3A4 & CYP2E1: Zopiclone and eszopiclone are extensively metabolized in the liver via CYP3A4 and CYP2E1 into N-desmethylzopiclone (weakly active) and zopiclone N-oxide (inactive).

06. Dosage and administration principles

Clinical Prescribing Mandate: Dosing must always be initiated at the lowest effective dose immediately before getting into bed. Never take a dose in the middle of the night unless at least 7 to 8 hours remain before waking.

  • Administration Timing: Take immediately before bedtime on an empty stomach. Taking with or immediately after a high-fat meal delays absorption and impairs hypnotic onset.
  • Dedicated 7-to-8 Hour Sleep Window: Do not take either agent unless you have a dedicated full night (at least 7 to 8 hours) to remain in bed before needing to be active or drive.
  • Initial Adult Dosing: For eszopiclone, the FDA mandates starting at 1 mg once daily at bedtime. For zopiclone, standard adult initiation is 3.75 mg or 7.5 mg once daily.
  • Geriatric Titration: In adults 65 and older, starting dose should not exceed 1 mg eszopiclone or 3.75 mg zopiclone to avert severe morning balance loss and confusion.

07. Common and expected side effects

The side effect profile of cyclopyrrolones is notable for an unusual sensory phenomenon alongside standard sedation:

SIDE EFFECTFREQUENCYCLINICAL PRESENTATION & COPING STRATEGY
Bitter / Metallic DysgeusiaVery Common (20%–35%)A lingering, bitter metallic taste upon awakening, caused by secretion of the drug and metabolites into saliva; rinse mouth with lemon water or brush teeth.
Next-Day Somnolence / HangoverCommon (10%–18%)Lingering morning grogginess; ensure a full 8-hour sleep window and avoid driving if drowsiness persists.
Dizziness & LightheadednessCommon (6%–12%)Cerebellar GABA modulation; rise slowly from bed to avoid postural instability.
Xerostomia (Dry Mouth)Common (5%–10%)Reduced salivary secretion; keep water bedside.
Anterograde AmnesiaOccasional (2%–5%)Inability to recall events that occurred after taking the pill; never take the medication before finishing all evening activities.

08. FDA boxed warnings and complex sleep behaviors

FDA BOXED WARNING: COMPLEX SLEEP BEHAVIORS RESULTING IN SERIOUS INJURY OR DEATH

1. Complex Sleep Behaviors: Complex sleep behaviors, including sleepwalking, sleep-driving, preparing and eating food, making phone calls, and engaging in other activities while not fully awake, have occurred after taking cyclopyrrolone hypnotics. These events can result in severe injuries, accidental burns, falls, motor vehicle collisions, and death.

2. Immediate Discontinuation Mandate: Discontinue zopiclone or eszopiclone immediately if a patient experiences a complex sleep behavior. These behaviors can occur at therapeutic doses, with or without concurrent alcohol use.

3. CNS Depressant Lethality: Co-administration with opioids (such as oxycodone or tramadol) or alcohol can cause fatal respiratory depression and unarousable coma.

Critical Warning Signs Requiring Immediate Emergency Care

  • Severe respiratory depression (breathing under 8 breaths per minute, gasping, or irregular shallow respirations).
  • Unresponsiveness, profound stupor, or inability to awaken the patient.
  • New or worsening depression, sudden suicidal thoughts, or uncharacteristic aggressive outbursts.
  • Severe allergic angioedema (swelling of the lips, tongue, or throat causing airway compromise).

09. Precautions and contraindications

CATEGORYCLINICAL CONTRAINDICATION & SAFETY RATIONALE
Absolute ContraindicationsPatients who have experienced complex sleep behaviors after taking any Z-drug; severe respiratory failure; myasthenia gravis; severe hepatic impairment.
Sleep Apnea SyndromeMay compromise ventilatory compensation during hypoxic obstructive episodes; contraindicated in severe untreated sleep apnea.
Pre-existing Depression & SuicidalitySedative-hypnotics can exacerbate clinical depression; closely monitor patients with mood disorders.
Pregnancy & NursingCrosses the placenta and passes into breast milk; may cause neonatal flaccidity and respiratory depression. Avoid unless potential benefits clearly justify fetal risk.

10. Critical drug and substance interactions

Hepatic biotransformation via CYP3A4 makes these agents sensitive to enzyme inhibitors and inducers:

INTERACTING SUBSTANCESEVERITYMECHANISM & CLINICAL OUTCOME
Alcohol (Ethanol)CONTRAINDICATEDDramatically escalates incidence of complex sleep behaviors, amnesia, motor coordination failure, and fatal respiratory depression.
Opioids (Oxycodone, Hydrocodone, Tramadol, Morphine)CONTRAINDICATEDSynergistic suppression of medullary respiratory drive; elevated risk of fatal nocturnal hypoventilation.
Potent CYP3A4 Inhibitors (Ketoconazole, Clarithromycin, Grapefruit)MAJOR CAUTIONBlocks clearance, doubling plasma concentrations and causing prolonged next-day impairment. Eszopiclone starting dose must not exceed 1 mg.
Potent CYP3A4 Inducers (Rifampin, Carbamazepine, St. John’s Wort)MAJOR CAUTIONDecreases eszopiclone exposure by over 80%, rendering the drug clinically ineffective.
Other Hypnotics & Sedatives (Zolpidem, Lorazepam, Carisoprodol)AVOIDRedundant central nervous system depression, profound amnesia, and severe motor ataxia.

11. Special populations & Beers criteria

American Geriatrics Society (AGS) Beers Criteria® Warning: Non-benzodiazepine benzodiazepine receptor agonist hypnotics (“Z-drugs” including eszopiclone and zopiclone) are categorized as Potentially Inappropriate Medications (PIMs) in older adults. They produce adverse events comparable to classical benzodiazepines, including elevated risks of falls, hip fractures, motor vehicle accidents, and acute delirium. First-line therapy for chronic insomnia in older adults is Cognitive Behavioral Therapy for Insomnia (CBT-I).

Pediatric Use Guidelines

Safety and effectiveness of zopiclone and eszopiclone have not been established in pediatric patients under 18 years of age. A clinical trial in pediatric patients with ADHD-related insomnia failed to demonstrate efficacy and reported significant adverse psychiatric events.

12. Patient safety checklist

14. Frequently asked questions

Why does zopiclone cause a bitter metallic taste in the morning?

Zopiclone and its active metabolites are secreted directly into the salivary glands after systemic absorption. The taste is not from tablet residue in your throat, but from saliva carrying the drug molecules. Rinsing your mouth, using mints, or brushing teeth can help diminish the sensation.

How does eszopiclone compare to zolpidem (Ambien)?

Zolpidem has a shorter half-life (approximately 2.5 hours) and is primarily effective for falling asleep. Eszopiclone has a longer half-life (approximately 6 hours), making it significantly more effective for maintaining sleep throughout the entire night and preventing early morning awakenings.

Can I take zopiclone if I wake up at 3:00 AM?

No. You should never take zopiclone or eszopiclone in the middle of the night unless you have at least 7 to 8 full hours remaining before you must wake up and be functional. Taking it with fewer hours remaining causes severe morning sedation, memory impairment, and hazardous driving deficits.
