1. Mechanism of Action & Therapeutic Use
Dual-action atypical opioid: weak mu-opioid receptor agonism combined with norepinephrine and serotonin reuptake inhibition (SNRI-like properties).
2. Prescribed Administration Guidelines
- Administer with or without meals; swallow whole with a full glass of water.
- Do not exceed 400 mg daily (or 300 mg daily in adults over 75 years).
- In renal impairment (CrCl < 30 mL/min), increase dosing interval to 12 hours.
- Gradually taper after prolonged use to prevent withdrawal symptoms.
Tramadol significantly lowers the seizure threshold. Convulsions may occur within recommended dose ranges, particularly when co-administered with serotonergic drugs (SSRIs, SNRIs, tricyclics) or opioids. May precipitate life-threatening serotonin toxicity.

Strict Contraindications: Children under 12 years; post-operative tonsillectomy in pediatric patients; severe respiratory depression; acute intoxication with alcohol/hypnotics; MAOI use within 14 days.
4. Adverse Reactions Profile

Serious Reactions (Urgent Medical Evaluation):
Seizures/convulsions, serotonin syndrome (hyperthermia, tremor, rigidity, tachycardia), respiratory depression.
5. Clinically Significant Drug Interactions
Contraindicated with MAOIs. Caution with SSRIs, SNRIs, triptans. CYP2D6 and CYP3A4 inhibitors alter active metabolite formation.
